The major problem is to decide the malignancy criteria of struma ovarii. Although ovarian metastasis of follicular thyroid carcinoma is extremely rare, the diagnosis of malignant struma ovarii depends upon the exclusion of the presence of carcinoma in the thyroid gland. Malignancy criteria, differential diagnosis and treatment of struma ovarii are reviewed whilst the presentation of a 52 years old woman with follicular thyroid carcinoma metastatic to the lomber vertebrea, left kidney and right ovary. Keywords : bone metastasis, well differentiated follicular thyroid carcinoma, malignant struma ovarii, ovary metastasis, renal metastasis
